How IVF Due Dates Differ

IVF due dates are more precise than natural conception dates because the exact fertilization date is known. For a Day 5 blastocyst transfer, subtract 19 days from the transfer date to get the equivalent last menstrual period (LMP) date, then add 280 days. For Day 3 embryos, subtract 17 days. This LMP-equivalent date is what your OB will use for all gestational age calculations. IVF pregnancies are typically dated by transfer date in the first trimester, then confirmed or adjusted by the dating ultrasound at 7-8 weeks.

IVF Pregnancy Timeline Differences

IVF pregnancies carry slightly different monitoring schedules. Most fertility clinics do an early ultrasound at 6-7 weeks (confirming heartbeat), a graduation ultrasound at 8-10 weeks, then transfer care to an OB. Because IVF pregnancies have higher rates of multiples and slightly elevated risk of certain complications (preeclampsia, gestational diabetes), many OBs monitor more closely with additional ultrasounds or blood work. Progesterone support typically continues through 10-12 weeks. The due date itself follows the same 40-week timeline as any pregnancy — the baby does not know how it was conceived.

⚡ CalcWolf Insight

IVF babies have the most precisely documented conception dates of any pregnancies in human history. Every embryo has an exact date and time of creation recorded in the lab. This precision makes IVF due dates accurate to within 1-3 days, versus a 2-week uncertainty window for naturally conceived pregnancies.

Frequently asked questions
Is an IVF due date more accurate than a natural one?
Yes — significantly. IVF due dates are accurate to within 1-2 days because the exact moment of embryo creation is documented in the lab. Natural conception dates rely on estimating ovulation from last period, which can be off by 1-2 weeks. Your IVF due date should only be adjusted if the dating ultrasound shows a discrepancy of 5+ days.
When do IVF pregnancies graduate to an OB?
Typically at 8-10 weeks after a successful heartbeat confirmation at the fertility clinic. Your RE (reproductive endocrinologist) will transfer your care with complete records. Most OBs are very familiar with IVF pregnancies and will schedule a follow-up within 1-2 weeks of graduation.
